Job Description:
Position Purpose summary: Provides therapeutic services to on-site clients, managing doctor’s orders, providing medical assessments, managing Nicotine Replacement and Opioid Agonist therapies. Providing client support and care from a medical background; working with program team, teaching health classes, adding to the holistic approach
Accountabilities:
Dispenses medications to clients (or to floor staff to monitor self-administration of clients as applicable)
Reviews physicians’ orders; arranges for treatments, medications and other therapeutic services as needed
Maintains unit records and clients’ charts according to established facility guidelines, and provide statistics and data for client care.
Distribute and administer nicotine replacement therapy (NRT) – if/when necessary
Evaluates clients’ care needs by assessing residents on admission, and on an ongoing basis
Evaluates care plans, judges appropriateness/effectiveness, and makes changes when necessary in consultation with the Program Coordinator
Collaborates with other members of the treatment team on client care needs
Facilitates educational groups to clients on health-related topics
Supports individual clients on issues relating to health promotion, medication and symptom management, and other health related subjects
Carries out interventions in the care plan, and administers direct nursing care
Carries out nursing procedures in accordance with the College of Nurses of Ontario Standards
Provides training to staff on health, infection control and medication related subjects
Follows MOH standards
Adheres to Toronto Harbour Light Ministries Policies & Procedures

MANAGERIAL/Technical Leadership RESPONSIBILITY:

FINANCIAL AND MATERIALS MANAGEMENT:
Responsible for medical supplies, client medications, various medical instruments
Responsible for ensuring all drug screening materials, urine cups and other are fully stocked
WORKING CONDITIONS:
Primarily office work, typical office setting, keyboarding, using computer screens
Some lifting/pushing/pulling of under 20lbs may be required

Education, Qualifications and Certifications:
BScN or an equivalent combination of training and experience
Must be able to be registered with the CRPO as a Registered Nurse or Registered Practical Nurse
Experience and Skilled Knowledge Requirements
Experience and education in substance use/concurrent disorders or mental health issues is preferred
Education, experience or equivalent in working with marginalized clients dealing with addictions, mental health, homelessness
Current registration with the College of Nurses of Ontario
Skills and Capabilities (examples provided below):
Knowledge and familiarity with computer operations and software applications, including client records
Experience working in an interdisciplinary, client-centered approach to care.
Excellent oral and written communication skills

Strong interpersonal skills
Vulnerable Sector Check: an original copy of a current Vulnerable Sector Check (VSC) that is less than six (6) months old is required by your start date, and the results of the check must be acceptable to The Salvation Army in its sole discretion. The cost of the VSC is borne by the employee.
Compensation:
The target hiring range for this position is $62,673.90 to $78,342.38 with a maximum of $94,010.85.Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
