About this role

Moorings Park is looking for a Physical Therapist to work in Home Health (Certified). The Physical Therapist is responsible for insuring and coordinating quality and safe delivery of home health physical therapy services following applicable licensure/certification requirements both State and Federal. The Physical Therapist has responsibility for the planning and therapeutic interventions of all physical therapy cases. Physical therapy will be provided by a licensed Physical Therapist or a licensed Physical Therapy Assistant, under the supervision of a licensed Physical Therapist, in compliance with the Plan of Care.

** $5000.00 SIGN-ON BONUS**

Schedule: Monday - Friday (Rotating Weekends)

This Physical Therapist will perform skilled therapy visits in Collier & Southern Lee counties.

Contributions:

  • Performs physical therapy assessment including evaluating the patient’s needs, including medical, psychosocial, and environmental needs.  When no skilled nursing is necessary, performs initial client assessment, obtains necessary authorizations, consents, and intake data.
  • Develops and implements, with physician, patient and the Clinical Manager, an individualized Plan of Care based on the patient’s physical, medical, psychosocial, and environmental needs, and makes appropriate revision as the patient’s condition changes. 
  • Establishes appropriate, realistic, and measurable goals in conjunction with the patients’/caregiver’s expectations, and then evaluates and document progress toward these goals. 
  • Supervises the Physical Therapy Assistant through:
    • Instruction on the initial care plan as established by the Physical Therapist.
    • Performing on-site visits and thorough review of documentation and communication,
    • Providing timely documentation of supervision of the Licensed Physical Therapy Assistant services. 
  • Supervises the Home Health Aide as appropriate:
    • Prepares the Home Health Aide assignment sheet/written instructions indicating care and tasks to be performed. 
    • Communicates regularly with the Home Health Aide
    • Performs and documents the Home Health Aide supervisory visits no less than every 14 days to:
      • Evaluate effectiveness of the Home Health Aide care provided.
      • Rapport with patient/family
      • Infection control procedures
      • Honoring patient rights
      • Following Home Health Aide assignment sheet
    • Revises the Home Health assignment sheet/written instructions as necessary. 
    • Instructs and supervises the Home Health Aide in new non-skilled tasks and procedures and documents such training. 
  • Completes a comprehensive OASIS Recertification/Follow-up Assessment along with other recertification documentation if Nursing is not in on the case. 
  • Completes a comprehensive OASIS Resumption of Care Assessment along with other resumption documentation, as appropriate, if nursing is not in on the case. 
  • Completes a comprehensive OASIS Discharge Assessment if physical therapy is the last skilled discipline providing service. 
  • The skilled services the Physical Therapist will provide may include:
    • Treating with active and passive exercises, muscle re-education, gait, and functional training.   
    • Administering therapy modalities as prescribed by physician.  Monitor plan for effectiveness.
    • Arranging for more continued Physical Therapy services, when indicated, with a Physical Therapy out-patient facility.
    • Observing and reporting patient’s progress or lack of progress to the physician.
    • Communication with interdisciplinary team regularly to coordinate care.
    • Education to the patient and/or family regarding therapy program and safety.  
    • Fiting, evaluating and adjusting adaptive devices.  
    • Prepare patient/family for discharge.
    • Documenting, accurately and timely, the patient’s treatment, care, evaluation, and progress, and communications with other staff members.
    • Participating in in-services QAPI and patient care conferences. 
    • Attending in-services and assumes responsibility for continuing education and maintaining current knowledge.
  • Obtains initial and interim physician's orders and reports on progress and changes in client condition to physician, Director of Clinical Services.   
  • Reviews and revises the skilled therapy care plan as needed.
  • Makes accurate and timely observations regarding the client’s care and health status.
  • Assures that accurate transcription and implementation of physician's orders have been recorded.
  • Participates in and contributes to continuing education in-services and attends CEU’s according to State guidelines for licensure.
  • Attends and participates in weekly case conference meetings. 
  • Attends and participates in In-service programs.  
  • Provides leadership and direction to ancillary personnel.
  • Maintains cooperative manner towards client/family and all members of the healthcare team.
  • Monitors documentation of discharge planning activities and assures discharge summary is written in timely manner.
  • Orients new personnel to policies, procedures, and documentation requirements of the HHA.  
  • Maintains client confidentiality and works in compliance with HIPAA regulations and standards.
  • Works in unison with others professionally and respectfully carrying out the Mooring Park Mission and Spirit Statements.
  • Demonstrates the desire to increase resident satisfaction.
  • Demonstrates the desire to improve employee satisfaction.
  • Shows initiative with respect to increased competency levels.
  • Assists Director of Clinical Services/Clinical Manager with hiring and training of new staff.  
  • Participates in health/job fairs to promote the home health agency. 
  • Assists Clinical Manager/Director of Clinical Services with employee performance evaluations by appraising work performance during supervisory visits.

Job Requirements:

  • Excellent oral and written communication skills in the English language required
  • During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
  • Graduate from a physical therapy curriculum approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.  
  • Meet CEU and HIV licensure requirements.  
  • Licensed as a physical therapist in the state of Florida.
  • Previous geriatric and home health experience preferred.   
  • Required to use own vehicle to travel to client homes. A current Florida driver’s license, clean driving record and proof of insurance are required.
