About this role

Family Care Network, Inc. (FCNI) is a mission-driven nonprofit serving children, youth, adults, and families across San Luis Obispo and Northern Santa Barbara Counties. Our team works collaboratively with local county and community-based agencies to provide compassionate, trauma-informed services; including evidence-based mental health treatment, case management, housing and homelessness support, and family-based interventions across Child Welfare, Behavioral Health, and Juvenile Justice systems of care—as well as support for individuals and families experiencing complex challenges who fall outside of these social services.

We are currently looking to add a new member to our dynamic team for the position of Case Manager in our Housing Support Program. In partnership with the Department of Social Services, the Housing Support Program (HSP) provides affordable housing for homeless families or those at-risk of becoming so. Program participants are provided necessary case management services for life skills development, one-on-one guidance, work group classes and community linkage to ensure their successful transition to self-sufficiency post care. The HSP Case Manager will provide one-to-one life skill development for HSP eligible participants.


Location: Remote (see below for remote provisions)

  • Employees living outside a 50-mile radius of the San Luis Obispo Main Office (SLO) will be assigned the nearest FCNI office for in-office workdays.
  • Direct Service Employees are classified as Remote, as their primary worksite is considered their home. Employees are still required to attend all meetings, supervision and client meetings in-person.


Anticipated start date: March 16, 2026

COMPENSATION

  • Pay Range: $23.00 - $34.50 per hour
  • Full Time, Non-Exempt classification per FLSA 
  • Spanish Bilingual differential (if applicable) - 10% increase on hourly base rate of pay
  • Mileage reimbursement $0.725 cents per mile


BENEFITS

  • Eligible to enroll into benefits on the 1st of the month following employee's first day of employment. These are all high quality and comprehensive plans.
    • FCNI will cover 75% of employee's premium and 60% of employee's dependent’s premium on the primary plan offered-Cigna PPO Health Benefit, Vision and Dental PPO 
  • 403(b) plan which allows employees to contribute a portion of their salary pre-tax toward their retirement with a monthly employer match.
  • 13 paid holidays annually
  • 17 days of PTO accrued annually (increases after 3 years of service)


EDUCATION & QUALIFICATIONS

  • Associate degree preferred, Bachelor's degree in Social Work, Psychology, or a related field preferred
  • One (1) year of direct experience in a Behavioral Health or Social Services setting such as outpatient clinics, facilitates and/or related community-based organizations, preferred
  • Must be computer literate with proficiency in Microsoft Office Suite and knowledge of database and reporting tools
  • Excellent communication skills (verbal, written, and interpersonal)
  • Excellent time management, organizational and multi-tasking skills
  • High level of accuracy and attention to detail
  • Ability to maintain discretion and confidentiality with sensitive information
  • Bilingual Preferred


RESPONSIBILITIES 

  • Interventions designed to improve client’s social and community competencies through building or reinforcing daily living skills and budgeting that will enhance the client’s ability to successfully locate and maintain housing in their community
  • Ability to work with clients with severe emotional needs in diverse settings.
  • Participate in treatment team meetings and agency specific case staffing
  • Prepare required written documentation, including case notes, incident reports, & agency forms
  • Participate in agency training, unit/staff meetings, activities, & events
  • Other duties and projects as assigned.
