As an Onboarding Specialist, you’ll play a key role in ensuring a smooth and efficient onboarding experience for patients. You’ll coordinate documentation, assign patients to the right internal teams, communicate with referral sources, and manage home health needs. This position serves as a single point of contact for designated tasks, teams, and referral sources.

What You’ll Do

  • Receive patient referrals via phone, fax, email, and/or EMR.
  • Document referral details and create patient information sheets.
  • Assign patients to the appropriate care team and update profiles.
  • Create patient entries for delivery management.
  • Enter and update progress notes and home health information.
  • Act as a liaison between internal teams and referral sources.
  • Manage incoming calls and process new referral markers.
  • Upload documents to patient medical records.
  • Organize and route electronic faxes to the correct destination.
  • Coordinate home health needs for patients.
  • Serve as the point of contact for assigned referral sources and therapies.
  • Assist with complex referrals and home health cases.
  • Train and mentor new team members.
  • Support intake nurses with faxing, copying, and distribution as needed.
  • Perform other duties as assigned.

  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel).
  • Strong organizational skills with attention to detail and multitasking ability.

Qualifications

  • High school diploma or GED required.
  • 2+ years of experience in a physician practice, ancillary provider, or similar healthcare setting.
  • Knowledge of medical terminology.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to prioritize and manage multiple tasks effectively.
  • Proficient computer skills, including Microsoft Office products.

As part of Evernorth Health Services, this role supports CarepathRx within 
our Pharmacy and Care Delivery organization. Our team focuses on 
specialty pharmacy and infusion services in partnership with hospitals and 
health systems.


If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.
