Qualifications:

  • Must successfully complete the required pre-employment Background and Drug Screenings   
  • Must possess a valid Driver's License which meets company standards for approved driver status                                                                                                                                                                                                                                                                                                                                                                
  • Must possess basic computer skills                                                                                                                      
  • Must possess a Bachelor’s Degree in the Human Services field                                                                                
  • Must have at least one year experience working in the field of intellectual/developmental disabilities                                      
  • Experience in writing program plans, case notes or other relevant documentation preferred  


Essential Functions:

  • Oversees and coordinates the Program Plan of each individual and ensures it aligns with the individual's needs and desires
  • Creates positive, collaborative communication with all disciplines and team members
  • Ensures that Program Plans and documentation are completed in accordance with all regulatory agencies
  • Assists in monitoring and training of staff and relevant parties to ensure accurate, implementation and documentation of training, support and behavior programs as required by the individuals’ plans
  • Responsible for assisting individuals to apply for third party benefits such as Medicaid and Medicare
  • Performs all other duties as assigned
