A Brief Overview
This Advanced Practice Provider (APP) provides advanced patient care in the acute care setting, based on educational preparation and/or certification in the practitioner role to a specific patient population. Care goals include promotion of healing, and prevention of illness and recidivism. The APP assumes the management of patients, functioning in both an independent and collaborative role with the physician, case manager, and other members of the health care team. The APP performs select procedures; designs individualized treatment plans, evaluates outcomes, and modifies the treatment plan as needed in accordance with established management protocols and regulating bodies. The APP facilitates the transition of patients between service units and provides/coordinates complex patients and family teaching.

The delivery of professional nursing care at Stormont-Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization.

Education Qualifications

  • Must be a graduate of an accredited PA/APRN program. Meets educational preparation or other qualifications within specified time frames as determined/outlined by specific department policy.


Experience Qualifications

  • 2 years Clinical experience as an APP in a related department is required to perform Lead APP duties. Five years of experience is preferred.


Skills and Abilities

  • All APRN incumbents are required to maintain Registered Nurse competencies as designated by Federal and State organizations and as outlined in SVH policy.
  • Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format. Ability to effectively present information to top management, public groups, and/or boards of directors.
  • Exhibits professionalism by assisting with research, educating others, and leadership in practice.
  • Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
  • Demonstrates and maintains expertise in the performance of selected procedures as specified by management protocols.


Licenses and Certifications

  • Registered Nurse - KSBN Must have valid APRN licensure. Be certified as a nurse practitioner by a recognized national certifying body that has established standards for nurse practitioners. Required or
  • Physician Assistant (PA) - KSBHA Required
  • Additional certification may be required in certain patient care settings or populations. The department director will provide specific information and the time frame in which such certification may be attained at the time of hire/transfer into department.


What you will do

  • Provides primary care for the trauma population in the inpatient and outpatient setting. Ensures that care delivered to patients is provided according to the Stormont-Vail HealthCare mission, strategic plans, and all internal/external regulations, policies and procedures. Practices within the Kansas Nurse Practice Act and within the guidelines of management protocols. Supports the established care models of Stormont Vail Regional Health Center. Evaluates patients in the outpatient clinic setting, monitors problem development and provides appropriate referrals for follow-up and problem resolution.
  • Functions independently and collaboratively with the physician and all disciplines to provide continuity of health care services for individuals, families, and/or groups. Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on history, physical exam, and psychosocial assessment in collaboration with other health care teams. May perform APP Lead duties as assigned including but not limited to schedule maintenance, coordinating coverage and PTO requests, monitoring assignments, assisting with performance evaluations and other clinical practice duties.
  • Analyzes data to identify health/illness problems, the human responses to actual or potential health/illness problems, variations from the norm in diagnosis, emergent problems, and complications. Communicates new or altered findings to the trauma surgeon.
  • Designs and evaluates individualized treatment plan. Coordinates care for continuity of care across the health care continuum. Collaborates with trauma services team to improve care and achieve verification. Under the supervision of the trauma surgeon, writes and effectively communicates orders in accordance with management protocols.
  • Attends trauma service rounds, participates in patient evaluation and care planning, reviews laboratory studies, vital signs and x-rays. Intervenes as determined appropriate in collaboration with the trauma team and within management protocols standards.
  • Incumbents will maintain bedside nursing skills by providing three flexible bedside nursing shifts per month and may be designated to work at the bedside for further shifts. Maintenance of skills work and assignment at bedside will be determined by leadership.
  • Dictates discharge summaries with physician co-signing. Provides early identification of discharge needs and ensures that appropriate referrals are made with case management and social services, nursing care team and attending RN.
  • Consults with other members of the health care team for emergency problems to expedite disposition/resolution.
  • Facilitates patient/family participation in health care by identifying learning needs and providing education to enhance decision-making. After discussion of the plan of care with the Trauma Surgeon, provides information to patients and their families regarding diagnosis, plan of care, expectations, and prognosis.
  • Assists staff in the coordination of a plan of care for the Trauma Services patients.
  • Refers Trauma Services patients to in-house support services and/or community services as needed.
  • Performs expanded role in the outpatient setting including suture, staple removal, wound care and other therapies in accordance with management protocols.
  • Provides telephone consultation to patients and/or physicians following discharge when on duty. Communicates with outreach physicians, hospitals, and EMS.
  • Has prescriptive authority per Kansas Nurse Practice Act and SVH protocols.
  • Assists in the development of research based clinical care protocols, guidelines, and pathways for trauma services patients. Facilitates changes in clinical practice based upon document research. Monitors and analyzes outcomes based on research driven practice change. Assists staff nurses in participating in research and incorporating the findings into their practice.
