Job Description:
Job Duties (but not limited to):
Help plan and lead activities that teach skills for jobs or community participation.
Provide personal care (e.g., repositioning, feeding, dressing, toileting) for clients.
Follow activity plans based on each individual’s needs to ensure meaningful activities.
Assist in creating schedules for individual activities.
Stay familiar with individual plans (IPP) and observe their progress during activities.
Work with the team to support the development of independence.
Collect and present progress reports to the team.
Record and analyze data to adjust activities as needed.
Train and guide staff and individuals in carrying out activities.
Transport clients to and from activities.
Ensure equipment and supplies are used properly for activities.
Order materials needed for activities.
Requirements / Qualifications:
Minimum Qualifications: Post high school coursework/training in human services or related field AND experience working with individuals who have a developmental or intellectual disability.
Preferred Qualifications: 3+ Years of working with individuals who have a developmental or intellectual disability.
Other: Valid Drivers License – 18 years of age or older. Incumbents are expected to successfully complete Basic Support class within 120 days and facility training requirements within 6 months of employment Specific positions in this class are required to obtain Mandt, CPR, and First Aid certification and successfully complete the training necessary to independently support residents in active treatment services. Specific positions in this class may require the incumbents to possess a valid driver's license and provide a passenger vehicle with adequate liability insurance, or the ability to provide independent authorized transportation, in order to perform work-related travel, such as transportation for clients.
Knowledge, Skills and Abilities:
Knowledge of:
Planning and leading activities for active treatment.
Labor laws, OSHA regulations, and relevant standards.
Intellectual and developmental disabilities, and mental health conditions.
Active treatment goals and behavior management.
Individual rights, curriculum methods, and training techniques.
Medicaid and state-specific regulations for active treatment.
Skills in:
Interpreting program plans and listening carefully.
Problem-solving, organizing work, and managing time.
Crisis intervention and communicating effectively.
Coaching, training staff and clients.
Observing and documenting treatment services.
Ability to:
Adjust programs to meet changing needs.
Keep individuals engaged and participating in activities.
Maintain records, compile reports, and assess progress.
Build strong relationships with families, community partners, and staff.
Teach staff and clients how to complete tasks.
Use computers and software (e.g., word processing, spreadsheets, and email).
