Saint Joseph's Medical Center is seeking a Team Leader to join our expanding Care Coordination program. This program provides support and coordination services to adults in the community who struggle with medical and mental health conditions. Reporting to the Program Director, the Team Leader will supervise a team of Care Coordinators who assist clients with concrete needs, including access to healthcare, entitlements and community supports.
Job responsibilities include:
Qualified candidates will have excellent communication and organizational skills.
Requirements: Master's degree in social work, psychology or a related field and 3 years of experience in a social service or health care setting, with at least one year in a supervisory role. At least one year of experience working with adults with mental illness is required.
Pay rate: $75-$80k

As a division of Saint Joseph’s Medical Center, St. Vincent’s also provides mobile crisis services for Westchester County, manages residential services for more than 1,300 individuals in Westchester County and New York City and operates three opioid treatment centers in New York City.
