JOB DESCRIPTION AND POSITION REQUIREMENTS

Scheduled Weekly Hours: 40

8 hr shift - Monday through Friday

JOB SUMMARY:

The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies.  In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations.  This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team).

MINIMUM QUALIFICATIONS:

  • LMSW or higher licensure required and three or more years hospital social work or protective services experience preferred
  • Working knowledge/experience in utilization management, managed care, and payor issues
  • Experience in psychosocial and therapeutic counseling
  • CCM, CMAC, or ACM preferred
  • Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.  Basic understanding of Medicare, Medicaid, and Managed Care regulations.  
  • Organized and efficient in establishing priorities and performing tasks within specific timeframes. 
  • Self-motivated requiring minimal supervision. 
  • Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress. 
  • Responsible for maintaining strict confidentiality of all verbal & written information and documents.
  • Familiarity with and appropriate use of community resources.

PHYSICAL DEMANDS:
Must possess good physical health.  Some requirements include but are not limited to standing, sitting or walking for long periods of time.  Lifting at least 20 pounds is required.

Frequently (34%-66%) - NONE
Occasionally (1%-33%) - lifting, carrying, pushing/pulling, climbing (stairs, ladders, etc.), balancing, stooping, crouching, reaching, handling/feeling
