Responsibilities

Under the direction and supervision of the Subacute Director, the Assessment Coordinator is responsible for conducting and coordinating the completion of the resident comprehensive assessment which includes the Minimum Data Set (MDS), and for tracking, and transmitting all data including the Resource Utilization Groups (RUGs) to the Department of Health and Human Services. The process includes the coordination of appropriate participating health professionals (interdisciplinary team) for the purpose of conducting initial and periodical comprehensive, accurate assessments of each resident to plan care that allows the resident to reach his/her highest practicable level of physical, mental and psychosocial functioning.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

 

  • Current and valid state Registered Nurse license OR Licensed Vocational Nurse license
  • Two (2) years of experience in a Minimum Data Set Nurse role with knowledge of the most current Resident Assessment Instrument principles in work application
  • Current BLS (AHA or ARC) certificate upon hire and maintain current.
  • Resident Assessment Coordinator Certification (RAC-CT) preferred.
  • Bachelor of Science in Nursing (BSN) preferred.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $42.37 to $59.25. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
