General Purpose of Job:  


The Birth Record Analyst is responsible fo preparing birth certificates by interviewing patients and abstracting other data elements from the medical record.  The Birth Record Analyst enters this data along with other maternal and infant information about the pregnancy, labor and delivery and post partum course into the State Vital Statistics System.  Provides individual paternity affidavit counseling.  Prepares paternity affidavits.  Prepares and mails complementary birth certificates. Has to obtain notary certification. 


Essential Duties:


This job description is intended to cover the minimum essential duties assigned on a regular basis.  Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.



  • Obtains birth information from parent(s) using appropriate worksheet

  • Reviews patient medical records and other resources, as needed, to obatin required birth information

  • Reviews birth certificate work sheet for completenss and accuracy

  • Enters birth certificate infromation into the State of Michigan's computer system

  • Validates information on birth certificate is accurate with the Mom including spelling of names, sex, birth time, etc.

  • Submit birth certificates with a 100% accuracy rate

  • Reports and transmits birth certificate in a timely manner as required by state law

  • Obtains signature(s) form parent(s) for birth certifcate

  • Completes and reviews paternity papers with parent(s) as needed

  • Changes baby's name in Sparrow's MPI utilizing STAR ( Epic effective 12/1/2012)

  • Works closely with the Obstetrical nursing staff when carrying out duties

  • Maintain valid Notary with State of Michigan

  • Works the unresloved list from Stat of Michigan by researching and providing the additional data elements in a timely manner

  • Working knowledge of the State of Michigan Birth Certificate and Paternity Laws with ability to communicate to patients in the appropriate manner

  • Understanding of the appropriate forms to be completed for Adoptions.

  • Maintain logs and report statistics Monthy ( example: sex of baby, method of delivery, information from prenatal, etc)

  • Work with RNICU caregivers to obtain necessary information for babys sent to unit

  • Completes Correction of Birth Certificate form, collects copy of mom's/ dad's driver license and submits to Operation Manager for Processing for birth certificates submitted in error.







Job Requirements





















General Requirements • Preferred - Certificated Medical Assistant
Work Experience • 3 years in a Medical Office setting • Preferred- Prior Experience with Birth Records
Education • High school diploma or equivalent • Preferred - Completion of Medical Assistant Program
Specialized Knowledge and Skills • Work Independently - self supporting, not needing to rely on others to complete job • Very Dependable and Responsible with Great work Ethic • Computer experience with various computer applications • Excellent communication skills both verbal and written • Ability to defuse uncomfortable situations in a positive and customer friendly manner • Must pass a typing test successfully at 40 words per minute. • Must successfully complete customer service skills assessment testing with 90% or greater. • Must successfully complete analytical skills assessment testing with 70% or greater.
