We are searching for a talented LPN Telehealth-VNA
Hours: 40hrs
Shift: Day shift, 8:00am - 4:30pm with weekend and holiday rotation
Location: Visiting Nurse Association - Fairhaven, MA

Responsibilities
Under general supervision of the TL-Telehealth Program or designee, perform duties to support and in support of the Telemonitoring department in an effort to reduce re-hospitalizations and provide proactive symptom management while adhering to best practices. Address data packets that are sent via the telehealth system with patients, field staff, physicians and clinic staff. Triage phone calls and provide patient education as it relates to data collected via telephone as needed. Troubleshoot equipment via telephone with occasional need for home visits to patients for equipment set up and or problems. Provide video visits in compliance with patient care plan.Qualifications
- Graduate of an NLN accredited school of Nursing (LPN).
- Current LPN Licensure in both RI and the Commonwealth of Massachusetts is required.
- Basic Life Support (BLS) and CPR certification is also required.
- Proficient computer skills, including experience with various software, database and electronic records programs is required.
- Two (2) years of related work experience is required; recent home care experience is preferred.
- Recent Telehealth and/or Cardio Pulmonary experience is preferred.
- Current LPN Licensure is required in both Rhode Island and in the Commonwealth of Massachusetts.
- Basic Life Support (BLS) & CPR certification is also required.
- Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired
Compensation: Pay rate will be determined based on level of experience.
