Southbridge Care Homes provides exceptional acquisition, management, and redevelopment services for long-term care homes and retirement communities across Ontario. Our mission is to maintain a best-in-class portfolio that consistently delivers excellence in care and services while ensuring economic sustainability.

Reporting to the Director of Care (DOC), the Restorative Care Aide enhances residents’ quality of life by implementing individual and group functional restoration programs based on assessed needs or Physiotherapist referrals. The role supports residents in maintaining and improving functional abilities to promote independence in activities of daily living, working collaboratively with the care team under the guidance of a Physiotherapist.

This is a full-time position with scheduled hours of 11:00 a.m. to 5:00 p.m., including a rotational weekend schedule and Wednesdays off. Extended hours may be required from time to time. The environment is fast-paced and deadline-driven, requiring strong attention to detail and effective time management skills. The role involves regular use of computer systems and collaboration with interdisciplinary team members.

Key Responsibilities:

  • Assist with group exercise programs, supporting residents with weight-bearing activities and guided range-of-motion (ROM) movements. Provide physical support to limbs or joints as directed, which may require crouching, bending, and reaching.
  • Provide one-on-one exercise and restorative care activities in accordance with individual restorative care plans. Facilitate ROM exercises and monitor resident participation and tolerance.
  • Conduct or assist with restorative assessments to evaluate resident progress and determine appropriate activities or exercises.
  • Support residents with Activities of Daily Living (ADLs), encouraging independence while providing guidance, assistive devices, and supervised practice. ADLs may include mobility, dental care, grooming, dressing, eating, and drinking.
  • Work with residents in various settings, including resident rooms, therapy areas, and dining rooms, based on care needs.
  • Assist with the restorative walking program, providing physical support using gait belts and safe mobility techniques. Guide and stabilize residents who may experience fatigue or unsteadiness. Collaborate with other staff when two-person assistance is required.
  • Maintain accurate and timely documentation, including daily charting and updates to care plans.
  • Participate in staff meetings, resident care conferences, and in-service education sessions as required.

Knowledge and Skills Required:

  • Occupational Therapy Assistant (OTA) or Physiotherapist Assistant (PTA) certificate or diploma required.
  • Physically able to meet the demands of the position, including assisting residents with mobility and exercise activities.
  • Demonstrated ability to work collaboratively within an interdisciplinary care team.
  • Strong communication and interpersonal skills.
  • Ability to follow care plans and restorative programs as directed by a Physiotherapist.
  • Knowledge of safe body mechanics and resident handling techniques.
  • Effective time management and organizational skills.
  • Basic computer skills for documentation and charting.
