Status: Full-Time Vacancy

Reporting to the Executive Director, the Director of Quality and Risk will be responsible for overseeing and driving quality improvement initiatives within the home.

About the Role:

You will assess the environment to identify opportunities for improvement that enhance resident care and outcomes. You will serve as the catalyst for quality and innovation processes, aligning all disciplines toward a common goal. Utilizing project management frameworks and tools, you will assess, understand, communicate, plan, execute, and evaluate quality improvement measures within the home.


Daily Responsibilities and Accountabilities:

  • Leads the RNAO Best Practice Guidelines program in the home.
  • Collaborates with the Director of Care with onboarding and performance management of nursing personnel.
  • Adhere to grievance and complaint procedures to address staff and resident concerns.
  • Conduct performance reviews to ensure that staff members meet job expectations and follow policies.
  • Collaborates with the Director of Care and Director of Clinical Services on special projects and reviews.
  • Access and implement project charters developed by others in the region.
  • Lead the evolvement and refinement of an assigned project charter.
  • Check in with other Southbridge Homes and the region regarding their implementation efforts.
  • Track and trend quality indicator performance monthly.
  • Update quality improvement spreadsheet with process and progress monthly.
  • Participate in monthly regional quality improvement calls.
  • Collaborate with other Southbridge QI leaders in the region to share successes, whether remotely or in person.
  • Attend and participate in preexisting committee meetings associated with BSC indicators.
  • Assemble a core QI committee and meet monthly (focus on frontline staff). 
  • Perform environmental scans to identify root cause problems and deficiencies.
  • Observe care and other practices (active observation vs passive observation).
  • Use various modalities to define and understand problems (5 whys, fishbone, spaghetti diagram, etc.)
  • Identify gaps in training and education that impact quality.
  • Introduce new care programs and services based on best practices.
  • Participate in nursing meetings on a regular basis to introduce new programs, policies, quality improvements, new initiatives, and changes in practices and policies.
  • Chairs Quality Committee and collaborates with the Director of Care and Director of Clinical Services any PSW Practice and Registered Staff meetings.
  • A member of the Leadership team, Quality Team meetings and other meetings /committees as assigned.
  • Oversee the Clinical Program of the Nursing Department: 1) Falls and Restraint Program; 2) Pain and Palliative Care Program; 3) Skin and Wound Care Program; 4) Continence Care Program; 5) Restorative Care Program; 6) Medication Management; and BSO team including and Continuous Quality Programs in the Home.  Conducts regular audits, analysis and plans of action to improve the quality and mitigate risk.
  • Attend other committees/teams’ meetings as requested or required.
  • Participate in facility strategic planning to align the nursing department in response to internal and external environmental challenges & Corporate Strategic Plan.
  • Ensure that policies and procedures are aligned with the facility's culture, vision, mission and values.
  • Assist in developing and sustaining the Quality Improvement program. In collaboration with the Director of Care and Director of Clinical Services, the interdisciplinary team develops, tracks, and evaluates quality and risk indicators.
  • Review and interpret clinical data for analysis and trends in quality improvement activities. Based on the identified trends and analysis, take the lead in changing practices and introducing new policies, programs, and services.
  • Provide the Senior Management with monthly Key performance indicators updates, including plans to address areas where targets are not met.
  • Assist with the evaluation of all resident programs and services for relevance, quality, and effectiveness
  • Partner with stakeholders, like colleges, universities, agencies, and professional associations.
  • Participate in industry benchmarking.

 

Qualifications and Competencies:

 

  • A minimum of 3+ years of Nursing supervisory experience, including a minimum of 3 years of experience in long-term care
  • Current certification with the CNO as a Registered Nurse (RN) is required.
  • Current license with the College of Nurses of Ontario.
  • Working knowledge and compliance with the accreditation Process and applicable Contractual & Legislative Requirements. CNO standards, FLTCA 2021, Act & regulations.
  • Ability to navigate PCC/POC documentation, reports and Insights (QIP), RAI-MDS knowledge, and Quality Improvement knowledge.
  • Strong knowledge and use of Microsoft Word, Excel and Outlook, Facilitation Skills, Recording and Reporting Skills.
  • Demonstrated exemplary leadership and communication abilities.
  • A positive outlook with a strong work ethic is required.
  • Deadline-oriented and strong prioritization skills are a must.
  • Be a coach and a mentor.
  • Effectively communicates with staff at all levels, residents, families, other departments, and external stakeholders.
  • Must have strong organizational skills and attention to detail.
  • Promotes the Person-Centered Care philosophy.
  • Demonstrates sound knowledge of the Resident Bill of Rights and monitors care to ensure the rights of residents are always respected and promoted.
  • Must be able to work flexible hours to meet the needs of the homes to work on QI initiatives or studies.
  • Adheres to Occupational Health and Safety practices.
  • Take every precaution reasonable in the circumstances for the protection of the worker.
  • Carry out all assigned duties in compliance with applicable Occupational Health and Safety legislation and Company policy.
  • Report any known or identified health and safety hazards or violations to their supervisor.
  • Refrain from any conduct that puts themselves, workers, contractors or others at risk.
  • Is aware of employees’ responsibilities and follows all Health and Safety policies and procedures.
  • Works safely to reduce the risk of injury to self, co-workers, and residents.
  • Is aware of WHMIS-regulated products and takes safety precautions accordingly.
  • Does not operate or use faulty equipment.
  • Promptly reports personal injury to immediate supervisor and seeks first aid as needed.
  • Participates in fire safety demonstrations and fire drills and knows the Home’s emergency plan.
