
Kalgoorlie (Full Time) Join our team in a vital leadership role supporting the delivery of high‑quality community care. Working Monday to Friday, 8am–4pm, you’ll mentor a team of 8–10 care staff and lead all care management activities for clients in the Support at Home Program, supporting individuals with low, medium and high care needs. A pool vehicle is available at times, and mileage for personal vehicle use is reimbursed.
Key Responsibilities Team Leadership
Lead, mentor and support direct care staff to ensure best‑practice service delivery
Manage performance, onboarding, communication and timesheet approvals
Promote a safe, positive workplace culture and ensure compliance with policies and procedures
Allocate resources effectively and respond to staff and client queries
Care Management
Coordinate all care planning and management for Support at Home clients
Conduct assessments and develop person‑centred care plans aligned with client goals and quality standards
Collaborate with clinical and multidisciplinary teams to address changing needs
Monitor care notes, service delivery and risks, escalating concerns when required
Support clients with My Aged Care reviews and help them understand their package budgets
Ensure accurate care management claiming and responsible budget oversight
The ideal candidate will have a strong background within the Aged Care Industry. In addition to this, candidates will be able to demonstrate the following experience, skills and attributes:
Under the Aged Care act 1997, successful candidates will be required to provide a satisfactory National Police Clearance prior to commencement
