We’re hiring a Clinical Liaison (Hospice) – a compassionate and collaborative professional who serves as an advocate for patients and families during the pre-admission process. This role bridges clinical, sales, and marketing teams to ensure a smooth transition into hospice care and supports the growth of our service line through strategic outreach.

What You’ll Be Doing



  • Serve as an advocate for patients from pre-admission through discharge from medical facilities

  • Collaborate with sales, marketing, and clinical teams to source hospice referrals from healthcare providers

  • Review Explanation of Benefits (EOBs) and consent forms with patients and families

  • Perform comprehensive nursing assessments to evaluate hospice eligibility

  • Support the Hospice service line in developing and executing sales and marketing strategies

  • Promote Silverado’s mission and values through community engagement and public relations


Qualifications



  • LVN or RN license preferred, but not required

  • Knowledge and experience working with hospice patients and families, professionally or as a volunteer, highly preferred

  • Effective communication and interpersonal skills

  • Must pass a criminal background check, physical exam, and drug screening

  • Valid driver’s license and clean driving record required

  • Must comply with all state and federal driving regulations

  • Reliable transportation required

Shift Schedule: Full-Time, Monday -Saturday, 8:00 AM - 5:00 PM

Anticipated pay range
$80,000$90,000 USD
