Overview

Responsible for treating complex diseases for patients of Sentara Healthcare. The Physician leads an interdisciplinary team of primary caregivers and administrative support staff to perform assigned duties while displaying the highest standards of ethical and moral conduct as well as acting in the best interest of Sentara Medical Group and Sentara Healthcare, supporting our mission, vision and values to optimize the health and well-being of our patients.

Residency and board certification must be in area of specialty. Board eligible physicians will be considered. Certification must be obtained within 4 years of eligibility.

Sentara Community Care was founded with the goal of developing innovative care models that not only address but also help reduce health disparities in under-resourced communities. We are looking for an individual who is dedicated to addressing community needs, fostering relationships, and promoting overall health equity. This Physician will serve as an Associate Medical Director/Clinician across our Woodbridge VA.

Position Highlights:

  • Location: Woodbridge, VA

  • Schedule: Monday-Friday varying hours based on Clinic/Mobile Clinic need, 1 Saturday per month for community event support

  • You will work for Community Care with shared schedule with SMG walk In Clinics

  • Experience as a licensed provider treating family medicine, pediatrics, and adults with complex issues

  • Shared call schedule, with RN first call

  • Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients health holistically.

  • Review quality and outcome metrics monthly with the healthcare team

  • Collaborate with local civic and faith-based leaders on community education programs

  • Dedication to addressing community needs, fostering relationships, and promoting overall health equity.
