Position Summary
The Certified Wound, Ostomy, and Continence Nurse (CWOCN) serves as a clinical expert and consultant in wound, ostomy, and continence care across a network of healthcare facilities. This remote role provides virtual assessment, care planning, staff education, chart review, and quality oversight while collaborating closely with onsite clinical teams.
The CWOCN ensures evidence‑based practice, supports regulatory compliance, and promotes optimal patient outcomes through proactive wound management and interdisciplinary communication.
Key Responsibilities
Clinical Consultation & Patient Management
Provide remote wound, ostomy, and continence assessments via telehealth platforms, EMR review, photographs, and digital communication.
Develop individualized evidence‑based care plans for pressure injuries, surgical wounds, moisture‑associated skin damage, ostomies, fistulas, and continence issues.
Review and interpret clinical data (labs, imaging, risk assessments, documentation) to support providers’ decision‑making.
Collaborate with physicians, NPs, PAs, RNs, and therapy teams to optimize patient outcomes.
Participate in care coordination and provide ongoing follow‑up recommendations.
Facility Support & Coordination
Serve as the primary remote WOC resource for multiple facilities.
Review facility wound logs, weekly wound reports, and risk matrices to identify trends and prioritize interventions.
Remotely support facility staff with complex wound case troubleshooting and ostomy supply planning.
Collaborate with leadership to address regulatory citations, survey preparation, and quality-improvement initiatives.
Education and Training
Provide virtual in‑services, competency sessions, and ongoing education on wound prevention, ostomy care, pressure injury staging, and best practices.
Mentor onsite wound champions or unit-based nurses to expand local expertise.
Develop digital training materials, guides, checklists, and reference tools.
Ongoing competency training for staff at service facilities.
Quality Assurance & Documentation
Conduct remote chart audits to ensure accuracy, regulatory compliance, and alignment with clinical standards.
Participate in facility wound rounds via video conferencing, reviewing documentation and providing recommendations.
Monitor progress of high‑risk wounds, pressure‑injury prevention programs, and skin integrity initiatives.
Assist in preparing quality reports, benchmarking data, and compliance audits for leadership.
Telehealth Operations
Utilize telehealth software, secure messaging, and EMR systems to conduct efficient remote consultations.
Ensure timely response to consults and maintain HIPAA‑compliant communication across facilities.
Participate in improving virtual workflows and digital wound-management processes.
Requirements
Active RN license in applicable state(s) – ideally Texas
CWOCN certification from the Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB).
Minimum 2–3 years of clinical wound care experience; multi-site or consulting experience preferred.
Strong understanding of wound staging, product selection, pressure injury prevention, and ostomy management.
Proficiency with EMRs, telehealth platforms, and remote photo-based wound assessments.
Excellent communication, critical thinking, and organizational skills and the ability to work independently
Must carry malpractice insurance
Preferred Qualifications
Experience in long‑term care, Rural health, home health, acute care, or rehabilitation settings.
Prior remote consultation or multi‑facility wound program management.
Familiarity with pressure-injury quality metrics, regulatory requirements (CMS, Joint Commission), and performance improvement programs.
Work Environment & Schedule
Remote position with flexible scheduling depending on facility needs.
May require occasional onsite visits for audits, training, or leadership meetings.
Involves virtual rounds, video‑based assessments, and regular communication with facility teams.
