About this role

SUMMARY

Support Long Term Care staff and residents by providing residents with close direct observation or 1:1 supervision. Will work under the direction of a Registered or Licensed Practical Nurse. Observation of residents include those who are at risk of harming themselves or others if left unattended.

SCHEDULE: 6:00am-2:00pm, 2:00pm-10:00pm,10:00pm-6:00am

MINIMUM QUALIFICATIONS

  • High school diploma, GED, or suitable equivalent preferred

RESPONSIBILITIES

  • Follow the resident’s plan of care.
  • Observe for changes in condition and report findings to the appropriate nursing staff
  • Must be knowledgeable and adaptable to understanding and responding to the emotional and cognitive state of the resident.
  • Obtains verbal report from resident’s nurse and Interdisciplinary Care Team to include information regarding the resident’s identified care needs, diagnosis, visual, auditory, or language barriers, as well as any special considerations.
  • Obtains instruction from nurse or Interdisciplinary Care Team regarding ambulation limitations and course of action for redirection of resident if needed.
  • Maintaining documentation.
  • Maintaining confidentiality of patient information.
  • Adhering to relevant policies, procedures, and standards.
  • Maintains a safe and clean environment.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
