POSITION TITLE: Licensed Practical Nurse

REPORTS TO: Nurse Manager

SUPERVISORY RESPONSIBILITY: None

DEPARTMENT: Clinical 

POSITION STATUS: Non-exempt

POSITION SUMMARY: The Licensed Practical Nurse plays a critical role in the delivery of quality medical care within the center.  This position requires a person with strong clinical skills along with assessment and communication skills.  The Licensed Practical Nurse provides care as described by the established policies and procedures of RCCHC.  This position will provide clinical care in collaboration with other partnerships to achieve positive patient outcomes.  The Licensed Practical Nurse will model behaviors consistent with the published values and the Code of Conduct of RCCHC.

POPULATION SERVED: Roanoke Chowan Community Health Center will provide open access, community-oriented primary care of the highest quality to all residents, migrants and seasonal farmworkers, as well as, visitors of Hertford, Bertie, Northampton, Gates, Washington and surrounding counties, regardless of their ability to pay.

 QUALIFICATIONS: 

  • Current license as an LPN in state of North Carolina required
  • BLS certification required
  • Work experience in a medical practice preferred
  • Must demonstrate competencies in nursing practice
  • Works effectively with others and is able to organize workload, as well as assume responsibility 
  • Must be able to work independently, exercise appropriate decision-making skills, and function effectively on a team
  • Must be able to speak and communicate clearly and effectively

 ESSENTIAL FUNCTIONS:

  • Perform independent nursing functions using RCCHC established procedures, policies, and guidelines.
  • Administer medications/immunizations as ordered by RCCHC providers, documenting accurately and observing patients for reactions
  • Answer and return patient phone calls and other inquires to ensure accurate and timely communication is facilitated between all members of the care team
  • Complete paperwork for assigned providers as per RCCHC guidelines
  • Acts as a secondary point of contact for urgent or “walk-in” patients or patients telephoning in with urgent care complaints
  • Establish reason for patient’s visit, completing the rooming process per established guidelines and documenting accurate data in the EHR  
  • Assist patients in the preparation for the provider’s examination, including undressing, assisting to the table, and dressing
  • Accurately review and record medications and /or procedures as directed by the provider
  • Accurately execute technical procedure when ordered such as EKG’s, drawing blood, obtaining and preparing laboratory specimens
  • Maintain proper storage and handling of equipment, supplies, and medications
  • Maintain a clean and orderly clinical working atmosphere; adequately prepare exam rooms and properly equip laboratory
  • Work with patients and family in giving accurate, understandable instructions in diet, special care procedures, medication, patient education
  • Participate as an active member of the RCCHC team
  • Maintain compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and accreditation

POSITION SUMMARY: The Registered Nurse is a point of triage for emergent or ‘walk in’ patients, or telephoning patients. Coordinates flow of patients in provider suite based on established and walk in patient schedules. Systematically collects patient data and communicates to provider to establish plan of care. Delivers patient care and performs treatments and procedures in accordance with the agency’s established policy and procedures. Initiates and participates in patient teaching. Demonstrates sound clinical judgment. Communicates effectively, presenting positive and professional image in all interactions. Demonstrates a commitment to Roanoke Chowan Community Health center’s Mission and Vision. Demonstrates a commitment to ongoing self-development and quality improvement. Maintains a clean and safe environment. Participates in achievement of organizational goals. Some extended hours may be required for evening clinics and weekend. The Registered Nurse will model behaviors consistent with the published values and the Code of Conduct of RCCHC.

  • Requires a current valid license as a RN in the state of North Carolina
  • Must demonstrate competencies in nursing practice and utilization of the nursing process.  
  • Must demonstrate the knowledge and skill necessary to provide clinical care.  
  • Must demonstrate knowledge of the principles of growth and development over the lifespan, and possesses the ability to assess data reflective of the patient’s status, and interpret the appropriate information required to identify needs of the patient relative to his/her age and condition. 
  • Must have effective communication, initiative, problem-solving ability, accountability flexibility, stress management, and basic computer skills.  
  • Must be flexible and willing to work at different locations as needed. 
  • Must have computer skills.
  • Must be able to work with all patient populations.
  • Works effectively with others and is able to organize workload, as well as assume responsibility and maintain confidentiality.
  • Must be able to work independently, exercise appropriate decision making skills, and function effectively on a team. 
  • Must be able to speak and communicate clearly and effectively.

  • Collaborates with provider or refers to Intake Guidelines to determine appropriate point of service.
  • Conveys information and coordinates follow up action plans at request of provider.
  • Coordinates flow of patients in provider suite based on established and walk in patient schedules.
  • Systematically collects patient data and communicates to provider to establish plan of care.
  • Delivers patient care and performs treatments and procedures in accordance with agency’s established policy and procedures.
  • Delegates tasks to lab, nursing, and clerical staff judiciously.
