About this role

The Associate Care Manager position reports to a Care Manager and is responsible to fulfill the relational, training and quality assurance functions for a group of Caregivers and Clients. This includes supervising Right at Home services and monitoring appropriate staffing and productivity. Facilitates and promotes healthy, positive relationships between client/family and the caregivers. Responsible for the reputation of Right at Home in all areas of influence. Works with the Care Manager and Corporate Services to help assure proper and ample training for Caregivers. This also includes compliance and adherence to all local, state & federal regulations. Excellent Caregiver and Client/Family experiences and interactions with Right at Home are foundational for excelling in this job.

ROLES AND RESPONSIBILITIES – Directly & Through Subordinates:

  • Performs all responsibilities of a Caregiver and the ability to be on call for last minute open shifts.
  • Ability to hold nightly and weekend rotation of Manager on Duty shifts throughout the week.
  • Knows when and how to call on the Care Manager and Corporate Services for assistance, input, backup on the day-to-day issues as well as emergency issues.
  • Responsible for Meet/Greet/Orienting Caregivers (at the client’s location) that are new to the client to introduce them to the client, orient them for the shift and ensure that the caregiver is prepared to deliver a great first shift, or delegates to the appropriate person.
  • Maintains good relations with Caregivers by setting clear expectations and keeping in constant communication.
  • Conducts new Caregiver interviews and Orientation.
  • Performs Assessments, at the request of the Case Manager, and completes the required documentation.
  • Ensures that all clients have the best care experience by maintaining an exceptional level of personal care through excited, well trained, professional caregivers and through performing monthly Supervisory visits, as requested by the Case Manager.
  • Keeps Corporate Services updated on any issues or concerns regarding clients, caregivers, Case Managers or Director of Nursing.
  • Responsible to complete ISP and assist Case Managers with Caregiver Interviews and Selection.

· Responsible for contributing to the overall mission of Right at Home & following all Core Values.

Perform other such tasks and assignments as directed by management/leadership of organization.

KNOWLEDGE, SKILLS AND ABILITIES

· Graduated from High School with your diploma. CNA experience required.

· Working knowledge of HIPAA.

· Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.

· Communicates on an intermediate level with others (internally and externally) to provide, exchange, or verify information, answer inquires, address issues, or resolve problems or complaints.

· Tracks, monitors, plans, coordinates, or schedules activities at an intermediate level to ensure deadlines or requirements are met.

· Ability to make recommendations to address and resolve issues.

· Ability to effectively communicate in writing, verbally and make presentations to internal and external staff.

· Ability to effectively deal with diverse groups and individuals and to maintain working relationships.

· Be proficient in preparing and proof-reading correspondence, forms,

· Proficient in Microsoft Office software: Excel, Word, PowerPoint, and Outlook.

· Ability to communicate effectively both orally and in writing.

· Knowledge and skill in records administration and maintenance.

· Ability to establish and maintain effective working relationships with others.

· Skill in developing office policies and procedures.


WORKING ENVIRONMENT

· Visits to Clients’ locations to train and assist with activities of daily living.

· Some administrative time at the field office.

TRAVEL

· Reliable vehicle and ability to drive at night.

· Must possess a valid driver’s license and up to date insurance.

· Normally, only local travel is expected for this position.

PHYSICAL DEMANDS

· If required to make patient visits, prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients.

· Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.

· Requires ability to handle stressful situations in a calm and courteous manner at all times.
