What You’ll Do
In this role, you will:
- Own: Providing in-office and virtual family practice visits, including evaluation, diagnosis, treatment, and follow-up care
- Support: Patients by managing acute and chronic conditions and educating them on preventive care and wellness
- Collaborate with: Medical Assistants, a Medical Director, and an interdisciplinary care team to ensure strong patient outcomes
- Improve: Access to primary care through a mix of in-person and telehealth services
- Advocate for: Patients by delivering thoughtful, patient-first care that removes barriers to treatment

What We’re Looking For
We’re looking for someone who:
- Active Arizona Nurse Practitioner (NP) license, required
- Must be located in Arizona with flexibility to work both in-office and virtually
- Family Practice or Primary Care experience, preferred
- Prior telehealth experience, a plus
- Strong clinical judgment, communication skills, and a patient-first mindset
- Tech-savvy and comfortable navigating EHR and telehealth platforms
- Bilingual (English/Spanish) preferred.
