<strong>About Qualicare Durham Region</strong> </div><div> </div><div>Qualicare Durham Region is a CARF-accredited, nurse-led home care organization providing high-quality, client-centred care since 2017, across the Durham and Central East regions. We are expanding our clinical services under a new Workplace Safety and Insurance Board (WSIB) contract and are seeking experienced Registered Nurses to support in-home assessments and care coordination for injured workers. <br><br>

<strong>Role Summary</strong> <br><br>
</div><div>The RN Assessor is responsible for completing in-home clinical assessments for WSIB clients, developing evidence-based care recommendations, and coordinating interdisciplinary services (PSW, RPN, OT, PT). This role is ideal for experienced community nurses who value autonomy, clinical judgement, and meaningful work. <br><br>

<strong>Key Responsibilities</strong> </div><div> </div><ul>
<li>Conduct in-home Initial Care Plan Assessments and Care Plan Reassessments</li>
<li>Perform comprehensive clinical, functional, and environmental assessments</li>
<li>Develop WSIB-aligned care recommendations and tier-level determinations</li>
<li>Complete high-quality clinical documentation and reports within required timelines</li>
<li>Communicate assessment findings and recommendations to WSIB case managers</li>
<li>Coordinate and oversee care delivery in collaboration with internal clinical leadership</li>
<li>Participate in case conferences as required</li>
</ul><div> <br><strong>Qualifications</strong><br> </div><ul>
<li>Registered Nurse in good standing with the CNO</li>
<li>Minimum 3 years community or complex care experience</li>
<li>Strong assessment, documentation, and clinical reasoning skills</li>
<li>Comfortable working independently in clients’ homes</li>
<li>Valid driver’s licence and reliable vehicle</li>
<li>Experience with WSIB, Ontario Health at Home, or rehab-focused care is an asset</li>

<strong>Compensation</strong><br> </div><ul>
<li>$60–$70 per hour (contract), based on experience and scope</li>
<li>Flexible caseload; volume expected to grow</li>
<li>Mileage reimbursement in accordance with WSIB travel rule</li>
