Job Summary 

The Hospice/Home Health Services aide's principal responsibility is providing personal care services according to written directions prepared by the RN or appropriate therapist and carried out under the supervision of the appropriate professional.  The Aide functions as an integral member of the Homecare team.  The written assignment for the aide is completed by the Registered Nurse in accordance with the written plan of treatment established by the physician indicating the patient's needs for Aide service.

Essential Functions

  • Must be able to read, write, understand, and carry out directions or instructions to keep simple records and is able to speak and understand the English language or the language of the agency's patient and/or staff member acting as the supervisor.
  • Ability to function with minimal supervision.
  • Good moral character and has never been convicted of a crime under the laws of this state or other jurisdiction.
  • Participates in case conferences if indicated and in service programs at least 75 % of the time.  Attends mandatory in-services 100% of the time.  Attends at least twelve hours of in-service a year related to Home Health/Hospice.     
  • Must exhibit great communication skills with the patient, family, and RN.  Supports teachings of all agency personnel and communicates changes in patient status to RN. 
  • Understands the physical and emotional and developmental needs and ways to work with populations served by Homecare.
  • Demonstrates proficiency in the following areas to an RN: 
    • Observes, reports, and documents patient status and care and maintains current and accurate clinical records.
    • Maintains a clean, safe, and healthy environment and maintains respect for patients and their property and families.
    • Basic infection control procedures.
    • Basic nutrition and food preparation.
    • Appropriate and safe techniques in personal care:  bathing, oral hygiene, shaving, feeding, and dressing (bath, shower, back rub, foot care, shampoo, skin breakdown, nail care, oral/denture care, making a bed, dressing, toileting, and assistive devices).
    • Safe transfer techniques and ambulation (out of bed, to a chair, walking assist, body mechanics, use of equipment, special techniques).
    • Obtaining vital signs.
    • Range of motion exercises and treatments.
    • May give medication reminders to patients but may not administer medications (removed line stating okay to administer medications.
    • Assists in activities of daily living, personal cares, basic elements of body functioning (bowel and bladder), positioning patient in bed in proper alignment.
    • Provides household duties to facilitate the patient's care at home and encourages patient conservation of energy to reduce symptoms.
    • Supports the patient and provides the appropriate setting to encourage safe environment.
    • Recognizes emergencies and knowledge of emergency procedures.
    • Provides additional services such as meal preparation, shopping, and/or family/caregiver respite to help the patient remain at home.       
    • Maintains patient confidentiality at all times.     
    • Regular attendance at the assigned work location is an essential function of the job.
  • Demonstrates ability to perform office duties which may include ordering supplies, managing equipment log/check out process, calculating and entering statistics from reports, entering patient information into our office electronic medical record system, and scanning medical records.

  • Competent in CPR.

Qualifications

  • Education: 
    • High school diploma or GED, preferred.
    • Must have a current driver’s license and auto liability insurance.
  • Experience: 
    • Completion of a 75-hour training program or other training program that meets requirements. 
    • Previous experience in hospital, nursing home, or home care preferred.
    • Must be 18 years of age.
    • Must be able to read, write, speak and understand the English language.
  • Computer Skills, preferred:
    • E-mail.
    • Microsoft Word
