Description

As a RN Case Manager, you will:

  • Visit with patients in their homes or nursing facility setting, providing assessment, teaching and skilled care
  • Work with and receive support of other professionals to problem solve and update the patient's plan of care
  • Other duties as assigned

Requirements

Qualifications:

  • Valid Registered Nurse license in the state of Missouri
  • At least two years of hospice/palliative care experience preferred
  • Excellent computer skills
  • Excellent organizational and communication skills a must
  • Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance
