\n- Full-time Hospice RN Case Manager position
- Schedule: Monday thru Friday, 8a-5p
- Available territory openings: Case Manager typically assigned to cover 1-2 counties near the Office.
- Performs physical and psychosocial assessment on all hospice patients.
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
- Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
- Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
- Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc

- One year Hospice experience preferred
- 2 years of RN experience required
- Licensed Registered Nurse in State of Indiana
- Current driver's license
- Must have reliable transportation and insurance
$75,000 - $85,000 a year
Salaried poisition + mileage reimbursement + Back-up On-Call Stipends (+visit pay if applicable)
