Expected pay for this position is $29.30 - $34.47/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
POSITION SUMMARY:
Responsible for identifying, assessing, planning, coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. Assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. The social worker assesses and intervenes to address the complex medical and clinical needs associated with illness, injury or chronic illness/ disease. Coordinates the health care team to facilitate and expedite the best outcomes for the patient and physician caring for that patient. Works with patient and treatment team to help establish goals and realize optimal outcomes as determined by current individualized care plans. Works with the Almost Home Kids leadership team to ensure alignment with the strategic goals and visions.
REQUIRED QUALIFICATIONS:
1. Bachelor’s degree in Social Work from a school or program accredited by the Council on Social Work Education.
2. Licensure: Shall comply with the Illinois state licensing and certification requirements.
a. Social Worker with a BSW who has completed 3 years of supervised clinical hours will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Illinois Department of Financial and Professional Regulation. Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Illinois Department of Financial and Professional Regulation.
b. Social Worker with a BSW who has not obtained 3 years of supervised clinical hours will be given the opportunity to complete this clinical practicum required to sit for the examination for practice of social work. Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Illinois Department of Financial and Professional Regulation.
c. Social Worker with an MSW who has not obtained their license will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Illinois Department of Financial and Professional Regulation. Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Illinois Department of Financial and Professional Regulation.
3. CPR Certification.
4. Ability to communicate effectively, in a positive manner and with empathetic approach, to patients/ families with a variety of health issues, age groups and socioeconomic backgrounds.
5. Care Management certification within 3 years of hire.
6. Working knowledge of a clinical EMR system and Microsoft Office applications.
7. Excellent decision-making and time management skills in order to work independently and efficiently with limited supervision.
8. Substantial interpersonal skills to develop and maintain cooperative working relationships with personnel from other departments.
PREFERRED QUALIFICATIONS:
1. Master’s degree in Social Work.
2. Chronic, complex illnesses pediatric care management experience.
3. Three to five years of experience caring for patients with progressively increasing responsibility. Broad knowledge of chronic illness and medical and community resources.
