Position Summary
Northwestern Settlement is seeking a Service Coordinator to implement the Youth Intervention Pathways (YIP) program in partnership with the Chicago Department of Family and Support Services (DFSS), the Chicago Police Department (CPD), and Chicago Public Schools (CPS). This role provides trauma-informed, strengths-based case management to justice-involved and school-referred youth ages 10–21, serving as a consistent, caring adult and connector to community-based services.

Key Responsibilities

Manage a caseload of up to 30 youth, referred by CPD and CPS.
Conduct needs and risk assessments; complete youth and family intakes; develop 90-day service plans; coordinate referrals; and complete follow-up and case closure.
Maintain consistent engagement with youth through phone, school visits, home visits, and meetings in community or neutral locations.
Work collaboratively with youth to identify strengths, needs, and goals, and support youth-driven decision-making.
Engage families as appropriate and connect them to supportive services (e.g., healthcare, housing, and other stabilization supports).
Communicate with referral partners within required timeframes and provide updates when youth circumstances change.
Enter and maintain accurate, timely data in Cityspan on a weekly basis to track outcomes and service progress.
Coordinate with Service Coordinators across service areas, as needed, to support continuity of care.

Compensation & Benefits

Compensation will be commensurate with experience and qualifications. In addition, all staff are equipped with the tools needed to succeed, including a dedicated work space, laptop, email, high-speed internet access, and all necessary supplies.

Qualifications
Bachelor’s degree in Social Work or a related field.
Experience working with youth ages 10-21, particularly youth impacted by trauma, violence, or system involvement.
Case management experience preferred.
Ability to work effectively with multiple partners and systems.
Strong interpersonal, communication, and organizational skills.
Bilingual Spanish (written and oral) strongly preferred.
Demonstrated cultural competence and a strengths-based, nonjudgmental approach.
