Job Description

New Wound Care Clinic in Glenview 

Full Time Days, no weekend no holidays

BSN and Wound Care experience highly preferred. 

 

The Clinical Nurse Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Responsibilities:

  • Provides education, guidance, and support to patients/families in order for patients to manage their therapeutic regimens and self-care activities to achieve maximum independence.
  • Facilitates access and provides rehabilitative services within the available patient resources, while being responsive to the needs of internal and external customers.
  • Assesses patient and family's current understanding of the management of the patient's therapeutic regimens and self-care activities.
  • Develops an individualized plan of care based upon nursing diagnoses/problems with consideration of patient's culture, age, and developmental level and sets appropriate goals.
  • Implements a plan of care to achieve established goals and continually evaluates patient and family response to the plan of care.
  • Provides patient/family education for the patient and family to manage the prescribed therapeutic regimens.
  • Coordinates with the team and scheduling team patient rounds.
  • Coordinates the discharge planning process to facilitate the transition to the next label of care.
  • Educates the team regarding patient's insurance benefits.
  • Reviews patient progress for medical necessity, guidelines for continued stay against performance, against protocol benchmarks/progress toward functional goals/appropriateness of level of service.
  • Communicates and conveys plans to referring agencies to facilitate a smooth transfer process.
  • Interfaces with Admission and PFS representative to verify/obtain on MANG status throughout discharge planning process as necessary.
  • Assures ongoing certification for continued stay, transition to appropriate LOC obtain pre-certification/authorization as required by payer for special services.
  • Communicate effectively with patients and their families using approaches appropriate to their age, cognitive status, language abilities and level of education and informs them of necessary information regarding their health status and rehabilitation progress.
  • Provides clear, concise and timely documentation regarding the nursing services provided and case management areas discussed.
  • Collaborates with internal and external case managers and coordinates services to optimize the patient's use of available resources to implement a successful discharge plan.
  • Serves as communication link between patient, family and the interdisciplinary team.
  • Participates in education and training sessions
  • Attends external continuing education programs and presents information to other staff.
  • Mentors new staff and students.
  • Performs other duties as assigned

Qualifications

Required:

  • Associate’s Degree
  • RN licensure in Illinois
  • CPR Certified
  • Applicable nursing knowledge to provide services to rehabilitation patients and their families
  • Ability to work independently within own discipline, yet practice within an interdisciplinary framework
  • Flexibility and ability to manage work in a changing environment.
  • Two years of nursing practice

Preferred:

  • Bachelor’s Degree
  • Case management skills
  • Two years of rehabilitation nursing practice
  • One year of case management experience

Sign-on Bonus Eligibility: Internal employees and rehires who left Northwestern Medicine within 1 year are not eligible for the sign on bonus. Exception: New graduate internal employees seeking their first licensed clinical position at NM may be eligible depending upon the job family.
