About this role

The RN Care Manager provides patient-centered care management services to patients and families within both the Emergency Center and inpatient settings. This position serves as an integral member of the interdisciplinary healthcare team and will be responsible for identifying care coordination needs in collaboration with the inpatient and ambulatory care teams, patient and patient’s external support system (as applicable).  Within the context of an interdisciplinary team, the RN Care Manager will participate in progressing patient towards discharge/next level of care by identifying barriers and implementing appropriate solutions/interventions. The RN Care Manager will work with patients and patients’ external support system to secure appropriate and safe transition plans in keeping with shared goals of care. The RN Care Manager will consistently and thoughtfully apply nursing values and continuous quality improvement in daily work.  Provide care management services to patients within both the inpatient and Emergency Center settings. No supervisory responsibilities of staff. May supervise RN students and/or assist in the orientation of new RN Care Managers. Position reports directly the RN Supervisor in Hospital Care Management. Works with physicians, nurse practitioners, nurses, other medical/surgical specialists and other Park Nicollet Methodist Hospital employees. Contact with patients, families and external community supports. 

Work Schedule: 
M-F, 8-4:30

Minimum Qualifications: 

  • Education, Experience or Equivalent Combination:
    • Associate’s Degree in Nursing with commensurate experience. 
    • Minimum of one year hospital-based med-surg nursing experience.
  • Licensure/ Registration/ Certification:
    • Current Minnesota Registered Nurse (RN) license
  • Knowledge, Skills, and Abilities:
    • Knowledge of the values, principles and methodologies of nursing
    • Ability to integrate social and medical data to make psychosocial assessments and implement appropriate treatment plans
    • Ability to establish and maintain effective working relationships with patients, families, colleagues, and community organizations
    • Sensitivity to cross-cultural issues 
    • Support and educate other disciplines when appropriate
    • Ability to work effectively as part of an interdisciplinary team 
    • Exceptional organizational and time management skills.
    • Incumbent must be self-directed and able to prioritize on a continual basis.
    • Ability to deal with change and ambiguity 

Preferred Qualifications: 

  • Education, Experience or Equivalent Combination:
    • Bachelor’s degree in nursing strongly preferred
    • Prior experience in any of the following areas: utilization management, case management, home care setup, discharge planning.
