POSITION: Linkage Specialist
DEPARTMENT: Pregnant/Parenting Women with Children (PPWC)
SUPERVISOR: Director of PPWC

POSITION SUMMARY: 

Under the direction of the PPWC Director, the Linkage Specialist will be working and managing the services between Parkland Addiction Team, their neonatal intensive unit, perinatal unit, the Nexus treatment team, and community methadone clinics.  It will be the essential duty of the Linkage Specialist to ensure pre-entry, during service, and post-services are coordinated between the clients and the agency. The Linkage Specialist will also provide education and servicing opportunities available through Nexus with additional area hospitals and clinics, in addition to linking services with our clients to the Delighted to Doula agency and MAT programs, in and outside of the DFW area.

ESSENTIAL DUTIES: 

  • Maintains frequent contact with the professionals at Parkland along with other area hospitals and clinics and with methadone clinics- in and outside of the Dallas County area with attention on providing brief program updates and maintaining satisfaction on the part of partner agencies.
  • Performs as a point of contact and treatment services between the client and Nexus staff during a client's hospitalization
  • Adheres to client confidentiality regulations and provide complete and accurate documentation of all services provided.
  • Provides needed transportation for clients (have a valid driver’s license and insurance).
  • Provides client supervision in a safe, therapeutic setting.
  • Utilizes and promotes the appropriate treatment model with all clients. Conducting appropriate interventions and providing support.
  • Observing and documenting relevant client information and behaviors. 
  • Provides group, family, and individual counseling and educational sessions.
  • Ensures communication lines between all departments (clinical, medical, administrative…etc.) are functioning optimally. 
  • Maintains accurate documentation in staff logs, incident reports, facility forms, and/or other reports promptly.
  • Documents notes and other clinical information in database systems including CMBHS and Welligent.
  • Escalates emergencies as appropriate.
  • Conducts and documents urine screening.
  • Maintains a valid driver’s license throughout employment with Nexus in the event they are called upon to provide emergency client transportation. 
  • Complies with all licensing and regulatory standards for medication storage, disposal, counting, reconciliation, etc.
  • Attend required staff training sessions by Nexus and state licensing requirements and completes all training on time.
  • Demonstrates regular predictable attendance and punctuality. 
  • Attends and participates in all staff meetings.
  • Adheres to and promotes the Nexus mission and values. Acts as a role model for others.
  • OTHER DUTIES WITHIN THE AGENCY MAY BE ASSIGNED.

EDUCATION AND/OR EXPERIENCE: 

  • High school diploma or general education degree (GED) required or equivalent experience. 
  • Experience with chemical dependency issues preferred.

CERTIFICATION, LICENSES, AND REGISTRATIONS:

  • Additional professional certifications and accreditation may be a plus for this position. 
  • Must remain current with professional licensing requirements and agency training requirements.
  • Current and clean driving record.
  • Completes all required Nexus and licensing agency training on time.

Language Skills:

  • Ability to read and interpret documents such as provider notes/orders, licensure standards, etc.
  • Ability to read and comprehend simple instructions, short correspondence, and memos. 
  • Ability to effectively present information to customers, clients, and team members in one-on-one and small group situations.
  • Ability to write routine reports and correspondence.

MATHEMATICAL ABILITY:

  • Ability to add and subtract two-digit numbers and to multiply and divide with 10’s and 100’s. 
  • Ability to perform these operations using units of American money and weight measurement, volume, and distance.
