Assessment Counselor
$7,000 Sign On Bonus!
Bachelor's degree (LSW or RN required) or Master's degree in Social Work or Counseling with licensure.
Shift:
- 12-hour shifts
- Full Time, Part Time, and PRN (Full Time: 3 12-hour shifts, with every other weekend rotation)
- Days and Nights
Hours: 7a-7p or 7p-7a
The Role Itself
- Coordinates care with interdisciplinary teams and external providers and updates treatment team on clinical information.
- Participates in treatment planning, including psychosocial assessments.
- Provides resource information and "warm-referrals" to community providers for engagement.
- Completes thorough, timely documentation in compliance with standards.
- Demonstrates crisis intervention skills and upholds confidentiality.
- Completes the admission process, including verification of benefits and necessary authorizations.

Education:
- Bachelor's degree (LSW or RN preferred), or Master's degree in social work or counseling with licensure.
- Must be 21 yrs or older.
