About this role

Hourly Pay Range:

$33.96 - $52.64 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

  • Position: RN Triage - Transitional Care Clinic

  • Location: 120 Spalding Drive, Naperville

  • Full Time 40 hours per week

  • Hours: M-F 8:00-4:30 – no evenings no weekends
    Required Travel: No

What you will do:

  • Room patient, record vital signs and brief history from patient.

  • Completes patient admission assessment, including database and medication list

  • Provides SBAR handoff for APP and pharmacist

  • Ability to work as assigned direct care nurse.

  • Completes patient discharge process, including After Visit Summary, and ensures medication reconciliation completion.

  • Accepts other RN responsibilities as requested by the Manager of TCC.

  • Reports to Manager, Transitional Care Clinic for initial and subsequent assignments.

What you will need:

  • Bachelors Degree Science - Preferred

  • 1 Year of telephone triage experience preferred, preferably in medical practice setting - Preferred

  • Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current State of Illinois Registered Nurse licensure - Required

  • American Red Cross CPR BLS or issued by American Heart Association - Required
