Coordinates and assists in the provision of Behavioral Health resources and services to clients.

Essential Functions:

  • Assists families in determining needed resources and services through a screening and triage process.
  • Answers questions and concerns of families and provides service referrals and scheduling.
  • Gathers referral and pre-admission information and completes all pre-admission data entry.
  • Identifies emergent or complex cases and seeks clinical review prior to finalizing the referral.
  • Collaborates with interdisciplinary teams to ensure efficient resource and scheduling services.
  • Utilize Electronic Medical Record and Scheduling software to assess the demand for services and the ability of current service capacities to meet the community’s needs.
  • Maintains confidentiality of all clinical and administrative information consistent with agency policy.


Education Requirement:

Bachelor’s Degree in psychology, social work or related field, required.

Skills:

  • Strong skills in customer service, process improvement, data entry, and advanced computer skills, required.
  • Proficiency with EPIC and the NCHBH system,preferred.


Experience:

Behavioral health or customer service experience, required.
