We are seeking a dedicated, engaging, and highly organized Service Coordinator to support residents across two senior communities. This position requires the ability to travel between both properties.
Bilingual preferred.

Position Summary
The Service Coordinator is responsible for coordinating supportive services, enrichment activities, and community resources that help residents maintain independence and a high quality of life.
Education
Bachelor’s degree in social work or a related field preferred.
Experience
Two or more years of experience in social service delivery, ideally within senior or family populations.
Strong working knowledge of community resources and supportive services.
Demonstrated ability to advocate, organize, problem-solve, and deliver consistent results.
Key Responsibilities
Coordinate programs, services, and activities for residents.
Provide support across both communities while following the assigned weekly schedule.
Assist residents with applications for support programs such as Medicaid, Medicare, and other benefit programs.
Organize and lead social events, engagement activities, and educational opportunities.
Engage residents daily to identify needs and provide appropriate referrals.
Serve as a liaison to community agencies and service providers, building a strong network of partnerships.
Identify affordable service providers and negotiate discounts when possible.
Develop and maintain a Resource Directory of state and local agencies.
Offer educational sessions on health, wellness, life skills, and available support services.
Deliver excellent customer service by supporting residents with basic needs and concerns.
Assist residents in accessing services such as transportation, meals, personal support, financial assistance, and more.
Document and report resident or family concerns as required.
Maintain positive and professional relationships internally and externally.
