
| This position coordinates and provides clinical pharmacy services for patients that range in age from birth to 100+ years. Provides pharmacy quality assurance, assists in development of policies and procedures and participation on performance improvement teams. This position is responsible for staff development and implementation of the pharmacy strategic plan for designated areas of responsibility. |
| (25%) Provides clinical pharmacy services including drug dosing, monitoring, and dispensing. Provides drug information and education to medical center employees, physicians and patients. Liaison for issues that arise to the pharmacy manager and director.
(15%) Monitors drug therapy for patients in their service line. Makes recommendations on the medication management of inpatients.
(10%) Works with all medical disciplines to improve patient care and decrease costs.
(10%) Represents pharmacy in formal and informal medical center meetings. Participates in pharmacy management and pharmacy team meetings. Implements the pharmacy strategic plan for designated areas of responsibility.
(10%) Develops policies and procedures for pharmacy and service line, and provides staff education. Ensures compliance with all department and system policy, all regulatory rules, and accreditation standards.
(10%) Develops new clinical pharmacy services, educates staff, and implements the new program.
(10%) Preceptor role for pharmacy students, interns, residents. |
(10%) Provides pharmacy staff coverage when needed. OTHER DUTIES AND RESPONSIBILITIES: Compounds IV and piggyback medications, TPNs, and narcotic drips. Makes all biohazard IVs, including IVIG/Gammagard, Remicade, etc. using aseptic technique and universal precautions. Follows USP 797 guidelines. Other duties as assigned.

Comments: Bachelor’s Degree in Pharmacy or Pharm. D. degree and current licensure in the State of Michigan required. |
| EXPERIENCE, TRAINING AND SKILLS: Three to five years of hospital experience required.
Basic knowledge of personal computer.
Analytical skills needed to interpret pharmacokinetic data and research drug information.
Written and interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.

Completion of an American Society of Health System Pharmacists (ASHP) accredited residency program preferred (post graduate year 1 and/or 2).
Board certification through the Board of Pharmacy Specialties required
PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS: Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.
