
At the direction of case management staff, this position assists with the implementation of the discharge plan as established by the discharge planner and care team to support timely coordination of care. The care coordinator must be able to collaborate with the discharge planner to proactively identify and initiate role duties to ensure timely facilitation of next site of care needs. This position provides excellent customer service to patients through timely follow up on post-discharge needs and ensures communication with the patient and multi-disciplinary team. All staff in this position will float across units, and assignments will vary based upon department needs.
| (80%)Based on patient preference, coordinates with outside agencies to include DME companies, post-acute care providers or specialty facilities, transportation agencies, and others as indicated. Ensures appropriate documentation is provided to post-acute care providers or specialty facilities, DME companies, and transportation services to include EMS as required.
(10%)Assists patients in scheduling post-acute follow up appointments including assisting patients to obtain a primary care provider when there is not one assigned.
(10%)*Maintains and manages knowledge of post-acute settings such as skilled nursing homes, home care agencies, infusion services, DME companies, hospice, acute rehab and long-term acute care facilities.
OTHER DUTIES AND RESPONSIBILITIES: Within scope of the role, uses critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast paced environment and take appropriate action.
Look up information in the computer to answer inquiries, documentation interactions and follow ups, and send referrals as indicated for the patient's needs.
Conducts phone calls and accesses systems to retrieve patient information for clinical care team members and sends appropriate information for placement or transfer. Electronically faxes internal referral information as indicated.
Works with internal and external resources to coordinate placement, discharge planning, and follow up care.
