About the Role:
We’re seeking an enthusiastic and experienced Grade 2 Physiotherapist to join our CART team on a part-time, ongoing basis (0.42 EFT / 32 hours per fortnight), working across multiple Monash Health sites.
In this role, you’ll be part of a dynamic short-term service supporting clients soon after their admission to acute or subacute care, while they await ongoing community physiotherapy.
You’ll work collaboratively within a multidisciplinary team to deliver best practice, evidence-based care, optimise operational effectiveness, and contribute to strong partnerships both within Monash Health and with external providers. A key part of the role involves providing education and support to clients and their families/carers to help them understand their clinical condition and confidently self-manage their symptoms at home.
With exposure across multiple Monash Health sites and home visits, no two days are the same. If you love variety, teamwork and seeing the immediate impact of your work, this role is for you.

CART - Intensive Support Team is a tiered approach to client care that allows resources to be directed to those who need them the most whilst actively supporting people in the community to avoid ED and acute presentations. Strategically CART partners with acute and primary care services to enable smooth transitions across settings. CART is person-centred and follows patients as their needs evolve in their continuum of care. CART provides short term care coordination for individuals with chronic conditions who are frequent hospital visitors and would benefit from improved self-management.
About You:
You are an experienced Physiotherapist with a passion for delivering short-term, client-centred care and supporting patients during their transition from hospital to community. You bring strong clinical assessment and treatment skills, combined with the ability to educate clients and carers to promote safe, independent living.
