About Monash Children’s at Home
Monash Children’s at Home (MC@H) provides holistic, individualised, and patient- and family-centred care for infants and children (newborn to 18 years) and their families, delivered within the home and community setting.
MC@H supports children with a wide range of conditions requiring ongoing specialised nursing care, including—but not limited to—medical, surgical, and oncology/haematology conditions. Our service is committed to delivering safe, high-quality care that promotes healing, independence, and family involvement outside the traditional hospital environment.
Our Services
Hospital in the Home (HITH) is the provision of acute care to patients in the comfort of the child’s home or another suitable environment. HITH is an alternative to inpatient admission where care can be safely delivered at home, following consent from the relevant treating medical or surgical team.
Children receiving HITH care remain inpatients of the hospital and are managed within a virtual ward model. Care is tailored to meet the needs of the patient and family and aligns with the care plan determined by the treating unit.
While at home, children are visited and supported by a team of experienced MC@H nurses and allied health staff, who maintain close communication with the child’s treating medical and surgical teams.
The Post-Acute Care (PAC) service supports children following discharge from an acute hospital admission. Care is delivered in the community and may include nursing and allied health support to promote recovery, safety, and continuity of care.
About the Role
We are seeking an experienced and enthusiastic Nurse Manager in a part-time (48hrs), ongoing basis to lead Monash Children’s at Home through a period of continued growth and service innovation.
As Nurse Manager, you will provide strong clinical and operational leadership within a dynamic community nursing environment. You will lead service development, workforce planning, quality improvement, and change management, while ensuring the delivery of safe, high-quality care to children and families in the home.
You will be recognised as a positive nursing role model and demonstrate a relentless commitment to patient safety, staff wellbeing, and service excellence. This role requires a deep understanding of the unique challenges of providing care beyond the hospital walls and the ability to build strong, effective relationships with patients, families, and internal and external stakeholders.
About You
You are an accomplished nurse leader who:
Has a passion for community-based paediatric care and service innovation
Is confident leading change and growth within a rapidly evolving service
Demonstrates strong leadership, communication, and organisational skills
Is committed to creating a culture of safety, accountability, and continuous improvement
Builds and maintains strong professional relationships with patients, families, and multidisciplinary teams
Enjoys working collaboratively across nursing, allied health, medical, and community services
What You Need
To be successful in this role, you will have:
A postgraduate qualification in one of the following:
Paediatric Nursing
Midwifery
Neonatal Nursing
Community Nursing
A minimum of five (5) years’ post-registration clinical experience
Recent and demonstrated management or leadership experience
Familiarity with Hospital in the Home and Post-Acute Care services, and/or
experience managing services in home and community-based environments (desirable)
