About the Role

We are seeking an experienced and motivated Grade 2 Social Worker or Occupational Therapist to join the Aged Persons Assessment and Treatment (APAT) Team within Monash Health’s Older Adult Mental Health Service. This is a fixed term parental leave position, until September 2026 and is 0.8 EFT.

 

This community-based role focuses on delivering high-quality, recovery-oriented mental health care to older adults (generally aged 65+) living in the Middle South catchment. Working as part of a multidisciplinary team, you will provide holistic assessment, treatment planning, therapeutic intervention, advocacy and care coordination to support consumers to remain living safely in the community wherever clinically possible.

 

You will work collaboratively alongside nurses, psychiatrists, registrars, neuropsychologists, social workers and occupational therapists to prevent relapse, reduce hospital admissions and promote quality of life using least restrictive approaches.

 

About Us 

 

The APAT Team delivers time-limited, intensive community treatment and plays a critical role in early intervention, relapse prevention and seamless connection to primary care and community supports. The Older Adult Mental Health Service provides specialist mental health care through:

 

  • Community-based services across Middle South and Dandenong regions
  • Acute inpatient services at Kingston Centre and Dandenong Hospital
  • Strong partnerships with Residential Aged Care Services and Psychogeriatric Nursing Homes

 

About You

You are a skilled allied health clinician who brings strong mental health expertise, sound clinical judgement and a collaborative approach to working with older adults, families and carers. You will demonstrate:

 

  • Experience delivering community-based mental health assessment and intervention
  • Ability to work effectively within a multidisciplinary team
  • Strong risk assessment, care planning and documentation skills
  • Commitment to recovery-oriented, person-centred practice
