Job Summary

The Pharmacist II - Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care to patients in a hospital setting. This role primarily functions in a decentralized clinical area, working autonomously with healthcare providers and patients on assigned units. Responsibilities include independent order verification, medication dispensing, therapeutic drug monitoring, patient counseling, participation in multidisciplinary rounds, adherence to sterile compounding guidelines according to USP 797/800, and teaching activities to support the PGY1 residency program. The Pharmacist II serves as a resource for other healthcare professionals and actively contributes to the advancement of pharmacy practice.    

Core Job Responsibilities

Decentralized Clinical Practice:

  • Provides advanced pharmaceutical care in assigned specialty units, collaborating directly with healthcare providers and patients.
  • Participates in multidisciplinary rounds and collaborates with the healthcare team to optimize patient outcomes.
  • Performs medication reconciliation and patient counseling.
  • Provides drug information and education to patients and healthcare providers.
  • Manages complex medication regimens and provides specialized pharmaceutical care.

Medication Verification and Dispensing:

  • Independently verifies medication orders, evaluating appropriateness of dose, route, frequency, and therapeutic indication.
  • Dispenses medications in accordance with established policies and procedures, maintaining accuracy and efficiency.

Patient Monitoring:

  • Monitors patient medication profiles for drug interactions, allergies, and contraindications.
  • Identifies and resolves drug-related problems.

Residency Program Support:

  • Precepts PGY1 pharmacy residents, providing guidance and supervision in clinical activities.
  • Develops and delivers educational presentations and lectures for residents.
  • Participates in resident evaluation and feedback.
  • Contributes to the development and maintenance of the residency program curriculum.

Sterile Compounding:

  • Adheres to USP and guidelines for sterile compounding, including proper garbing, aseptic technique, and environmental monitoring.
  • Prepares sterile IV admixtures and other compounded sterile products accurately and safely.
  • Provides training and oversight to pharmacy technicians and other staff involved in sterile compounding.
  • Participates in the development and implementation of sterile compounding policies and procedures.

Other Responsibilities:

  • Mentors and trains pharmacy students.
  • Participates in the development and implementation of departmental policies and procedures.
  • Contributes to quality improvement initiatives and medication safety programs.
  • Maintains current knowledge of pharmacy practice and new drug therapies.
  • Adheres to all New York State and federal regulations governing pharmacy practice, including those outlined in the Education Law and Part 63 of the Commissioner's Regulations.
  • Performs other duties as assigned.
     

Education/Experience Requirements

REQUIRED:

  • Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
  • 1-3 years of experience as a hospital pharmacist.
  • Demonstrated ability to function in a clinical area (e.g., emergency department, intensive care unit, etc.).
  • Excellent clinical skills and knowledge of pharmacotherapy.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Demonstrated commitment to patient care and safety.
  • Interest in teaching and precepting pharmacy residents.
     

PREFERRED:

  • Completion of a PGY1 pharmacy practice residency. 

Licensure/Certification Requirements

REQUIRED:

  • Current pharmacist license in good standing in New York State.

PREFERRED:

  • Board Certification from the Board of Pharmaceutical Specialties.
