Human Service Coordinator

Department: Community Reintegration Center

Full-time, Salary

Salary Range: $49,753.60 - $74,630.40/yr

 

Job Summary

The Human Service Coordinator (HSC) will conduct assessments and treatment plans in addition to scheduling FAM center visits. The HSC will also coordinate with community partners to address barriers such as vital documents and other reentry resources.  

 

Work Schedule: This position will work the day shift Monday-Friday.

 

Job Responsibilities

  • Responsible for Level of Service/Case Management Inventory (LS/CMI) Assessments - All sentenced residents that have 90 days or more must have an LS/CMI assessment. Provide monthly written and oral reports regarding the assessments.
  • Create Treatment Plans - Responsible for creating all treatment plans for sentenced residents that have completed an LS/CMI assessment. Provide monthly written and oral reports regarding treatment plans.
  • Schedule FAM Center Visitations - Provides monthly written and oral reports regarding family visits. Coordinates and is responsible for all aspects of the setting up the FAM Center visits with the family and resident.
  • Work Collaboratively with Community Partners - Coordinates with other agencies to obtain vital records or necessary documents for reintegration such as housing referrals. Also, this position may be required to attend offsite meetings.
  • Ability to lift, carry, push and pull up to 50 lbs.
  • Other duties as assigned.

 

Required Education, License/Certification and Experiences

  • Current residency within the State of Wisconsin upon application required
  • Valid Wisconsin Driver’s License required upon application and must be maintained throughout incumbency
  • Bachelor’s degree or higher in Human Services, Social Work, Psychology or closely related field required
  • One (1) year or more of work experience in Human Services specializing in Mental Health, Children’s Disabilities or Family Counseling required
  • Basic knowledge of all related computer and software applications such as Microsoft Word, Excel, Outlook, PowerPoint and Teams required
