The Transfer Center Coordinator will ensure smooth system-wide patient placement, streamline transfer processes both into and out of all hospitals, and advance the seamless operation of clinical decisions related to patient movement. The transfer center coordinator will work with the patient placement nurse, room control associate, registration staff in collaboration with other departments and clinical leaders. Ensure optimized transfers both into and out of all hospitals across the system. Including internal transfers, upgrades in care and external transfer requests both into and out of the system. Includes working closely with care coordination, outside facilities, EMS partners and other community and statewide resources on a daily basis, as well as in times of disaster or diversions, over crowding or any other events as they may arise.
