Overview

Performs specialty functions in support of medication reconciliation. Obtains a current medication list for admitted patients through in-person interviews. Performs second sourcing by collecting medication list and verifying this with a second source. 

Qualifications

Education:

  • High School Graduate or equivalent required.

Experience:

  •  Minimum of six months experience in healthcare preferred.

 

Other Knowledge/Skills/Abilities:

  • Demonstrates excellent verbal communication skills.
  • Demonstrates excellent interpersonal skills.
  • Ability to input data accurately and efficiently into the Cerner Health Information System.
  • Effectively utilize drug information resources.
  • Maintain confidentiality of patient information.
  • Ability to set priorities.

Responsibilities

  • Conducts scripted interviews with patients admitted to the hospital.
  •  

  • Obtains the patient’s most accurate and current home medication list, including prescription and non-prescription medications.
  •  

  • Obtains the patient’s medication allergy history.
  •  

  • Resolves home medication list discrepancies with an in-patient hospital pharmacist and/or an MMC approved second source of information.
  •  

  • Accurately documents the home medication list and allergy information in the Cerner Health Information System.
  •  

  • Performs all other duties as assigned.
