About this role

Responsibilities:

Job Summary: The Patient Logistics Coordinator is responsible for ensuring accurate and timely patient assignment and transportation based on needed service(s), level of care and equipment.

  1. Responsible for coordination of transportation of patients, including alternative modes as needed.

    1. Completes daily checks for pre-scheduled trips to confirm appointment and pick up times.

    2. Coordinate ambulance transfers from the McLeod Health hospitals to other facilities and post-acute care providers.

    3. Monitors projected discharges and transfers per unit daily.

    4. Utilizes appropriate resources for transport, i.e., Heart Reach, air medical, contracted providers.

  2. Responsible for the placement of inpatient/observation patients, following the review of admission/discharge/transfer (ADT) orders and bed requests, in conjunction with physician documentation.

  3. Works with Transfer Coordinator to manage patient admissions, transfers and placement within the capacity and capabilities of each facility/unit.

    1. Utilizes his/her knowledge and expertise to facilitate safe and efficient patient throughput focusing on placing the right patient in the right bed at the right time.

  4. Coordinate and strategize the complex placement of incoming admissions, inter-hospital transfers, and post-operative patients in accordance with hospital policies, using EPIC Bed Planning software.  

  5. All other duties as assigned.


Qualifications:

  • Minimum of 1 year healthcare experience or training.
