Job Summary
Under general direction of the Pharmacy Specialists for Operations and Clinical Services, the Per-Diem pharmacist performs a variety of professional level functions to assist in providing patients with optimum pharmaceutical care. The pharmacist's efforts are directed primarily toward reviewing medication orders, conducting interventions with nurses and physicians and overseeing/checking the work of technicians, including the dispensing of medications and pharmaceutical preparations. The pharmacist participates in the quality improvement efforts of the department. The pharmacist participates on patient care teams, including medicine, surgery, pediatrics/oncology, sterile products, etc. Serves as a member of the code call team by preparing medications and providing information; inspects and monitors medication orders and the dispensing of medications to the patient care units; prepares continuous narcotic infusions on the nursing units; evaluates and investigates orders for non-formulary drugs and makes recommendations regarding therapeutic alternatives; interviews patients to evaluate the severity and significance of adverse drug reactions (ADR) and allergies; documents ADRs, participates in medication reconciliation
