SUMMARY

Under the supervision of the DRS management team, the Coordinator oversees daily operations of DMHAS Community Support Services, including the Young Adult Services (YAS) cluster and other community-based cases. The role is split between administration and direct case management, focusing on trauma-informed, recovery-oriented programming to support independent living. Responsibilities include supervising staff, managing scheduling, ensuring compliance with agency and state standards, and collecting required data. The Coordinator also handles hiring, payroll, and training to ensure staff follow Marrakech policies and ARC principles while delivering individualized support services.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Develops and implements Individual Plans (IPs) for assigned caseload within DMHAS timelines, collaborating with clinicians and team members to ensure consistency in service delivery.
  • Maintains up-to-date, audit-ready case and medical records; ensures timely submission of all reports and documentation, including DDAP and Service Delivery data.
  • Coordinates care using the CLAMP model (Coordinate, Link, Assess, Monitor, Plan) and ensures staff follow the same approach. Provides direct support and supervision to staff, particularly regarding life skills instruction aligned with LIST curricula (e.g., hygiene, budgeting, meal prep, decision-making).
  • Schedules and facilitates monthly and quarterly Dream Team meetings, preparing and submitting related reports and minutes.
  • Conducts regular audits of documentation systems (ETO, Red Medical Book), and teaches staff best practices for clinical record-keeping and data entry.
  • Provides 1:1 support and crisis interventions; implements behavioral, safety, and sensory support plans. Ensures all health needs are met including scheduling appointments, medication administration, and follow-up.
  • Leads trauma-informed, person-centered community integration activities tailored to individuals' developmental goals and schedules.
  • Ensures supervision and care activities meet prescribed standards (e.g., food prep for special diets, adherence to eating procedures, active treatment).
  • Maintains a visible presence at the Whalley building and Chill Zone, modeling trauma-responsive and clinical best practices.
  • Attends core and Dream Team meetings, conducts assessments, and contributes to plan development based on results (e.g., FBA, LIST, risk assessments).
  • Provides accurate, timely documentation (case notes, incident reports, logs, etc.); supervises Assistant Case Managers in doing the same.
  • Drives safely for work-related duties, using personal vehicle when necessary.
  • Acts as an essential staff member in scheduling, supervision, and program implementation across all service areas.

EDUCATION and/or EXPERIENCE

Bachelors’ Degree in a related field (with relevant classroom and internship experience). Master’s degree and supervisory experience in related field preferred. At least two years of demonstrable, relevant experience in a residential or community setting with clinical experience carrying out treatment plans in residential or other community settings. At least one year of supervisory experience. Familiar with and completion of ARC trauma informed practices curriculum.

SALARY

  • $44,766/yr. Rate increases to $47,989/yr after 6 month introductory period.
