The Supported Housing Case Manager is responsible for oversight and coordination of the CoC (Continuum of Care) Program. This employee will advocate and assist individuals with SMI and whom are literally homeless to over-come barriers to obtain and maintain safe & affordable housing. They will assess individuals’ strengths & needs, as well as coordinate supportive services with other program staff, assist individuals with accessing community resources, and build & maintain professional relationships with community partners. They will also support other Housing Team Staff, and fill in where needed.

DUTIES AND RESPONSIBILITIES:

Provide ongoing needs assessments for clients.
Participate in developing treatment plans to ensure clarity, measurability, and applicability to the client to promote self-sustainability.
Provide ongoing assessment and encouragement in regard to clients Activities of Daily Living (ADL’s) to promote self-sustainability.
Complete all documentation timely and accurately in the format approved by funding sources and regulatory statutes.
Serve as Liaison with KHC for the CoC Program
Act as liaison between client and community; building relationships with families and cooperating entities to develop additional resources and coordinate services.
Must have transportation and be willing to assist clients in meeting their needs by providing outings, transporting to scheduled appointments with various community advocacy agencies, etc.
Assist clients in managing and maintaining appointments and ensuring participation to prevent further hospitalization
Other duties as required/assigned by Team Leader and/or supervisors.
Assist with all Supported Housing Programs when needed
Requires travel within 10-County service area.

 

JOB REQUIREMENTS / EXPECTATIONS:

This position is equal time between client interaction and documentation.
Transporting clients via personal vehicle or company vehicle
Primary location is Corporate Office in Warren County in a clinical setting
General knowledge and understanding of computers, and willingness to learn new systems.

QualificationsQUALIFICATIONS:

Bachelor’s degree required, Preferably in Psychology, social work, sociology or human services
Valid drivers license, insurance and reliable transportation
