Job Summary

Using interviewing, organizational and networking skills, gathers clinically relevant information and provides psychosocial assessment to assist the healthcare team developing a treatment plan for the patient. Actively collaborates with healthcare team to meet patient care outcome management and system improvement goals. Uses planning, prioritizing and organizational skills to achieve utilization and quality goals.

Minimum Qualifications

Minimum Education: Master’s Degree in Social Work (MSW) from an Accredited Program
Minimum Years of Experience: None.
Substitutable Education & Experience: None.
Required Certifications/Licensure: Licensure as an Independent Social Worker-Clinical Practice (LISW-CP) in the state of South Carolina
Required Training: Experience with psychological assessments (i.e., personality assessments, depression assessments), evaluation, and treatment planning

  • Abides by the Social Work Code of Ethics.
  • Conduct psychological evaluations and provide recommendations to the clinical team from a mental health perspective 
  • Coordinate, lead and facilitate behavioral workshops.
  • Counsel, coach and educate patients and family on lifestyle changes and setting goals to promote behavior change.

Duties & Responsibilities

  • Provide support to patients and education to team members and staff.
  • Lead evening support groups with other team members.
  • Documents assessment information in the patient’s medical record (EMR).
  • Conveys and receives information efficiently to and from patients, families, other members of the health care team and outside facilities and agencies.
  • Provides education/consultation to the patient/family and healthcare team.
  • Determines comprehension and need for repetition and revision of methods of teaching.
  • Uses appropriate channels for reporting progress and/or concerns.
  • Consults with attending provider to coordinate care plans for the patient.
  • Identifies opportunities for improvement and coordinates/participates in the development and implementation of action plans to make improvements.
  • Contributes to patient outcome measurement.
  • Participates on teams designed to improve systems based on analysis of data and evaluates   outcomes.
  • Participates in interdisciplinary patient care conferences
  • Performs all other duties as assigned by authorized personnel or as required in an emergency (e.g., fire or disaster).
  • In addition to the above duties, Licensed Social Workers assigned to the Bariatric Program will perform a variety of duties to support and in support of Bariatric Services including contributing to research, quality initiatives and the development of curriculum and services for the behavioral aspects of weight loss programs.
  • Participates in Bariatric Team meetings while acting as a patient advocate. 
  • Develops Bariatric program to assist with maintaining program accreditations.
