Essential Functions: The Care Manager Associate provides coordination of health care and related services to infants, toddlers, school age children, adolescents, young adults, and their families for home & community-based services. Develops the plan of care with the client/family and interdisciplinary team and ensures client satisfaction with the services.
EXPERIENCE QUALIFICATIONS
- Required: Case management experience in an outpatient setting.
- Preferred: 1-2 years Pediatric case management experience.
EDUCATION QUALIFICATIONS
- Required: Bachelor's or equivalent in mental or public health related field required.
- Preferred: Master's or equivalent in public health related field, criminal justice, psychology or related field.
KNOWLEDGE, SKILLS, AND ABILITIES
- Excellent people skills, using and oral/written communication.
- Great computer skills and the ability to learn new computer programs.
- Excellent documentation and time management skills, ability to prioritize important.
- Must be able to work as an individual contributor and also contribute to teamwork.
- The ability to organize work is essential.
